Thanks for your interest in the Blood Pressure Wellness Program! We’re excited to work with you to lower your blood pressure and risk of heart disease.

The goal of the program is to help you build healthy lifestyle habits so you can reach (and maintain) a blood pressure of 140/90 or less. The program will complement any medications that your doctor or nurse practitioner might prescribe.

What to expect

The Blood Pressure Wellness Program includes 5 sessions with a health guide. Your sessions will happen over 2 to 3 months. Each session will cover a different topic related to lowering blood pressure:

We’ll review the basics of blood pressure, discuss how to take an accurate reading at home, and start the conversation about lifestyle changes to lower your blood pressure.

What we need from you

To complete the program and get your $25 gift card, you'll need to:

  • Participate in all 5 sessions with your health guide
  • Review the assigned resources above before each session
  • Complete the “on your own” home activities we give you
  • Bring questions and be ready to actively participate
  • Take a survey before and after the program
  • Check your blood pressure at least twice a month
