Thanks for your interest in the Diabetes Wellness Program! We’re excited to work with you to manage your diabetes, lower your risk of complications, and support your long-term health.
The goal of this program is to empower you to effectively manage your diabetes and improve your A1C (your average blood sugar level over the past few months), which tells us how well your diabetes is being managed. Our approach complements the care you receive from your doctor or nurse practitioner and is built on the core principles of the American Diabetes Association's Diabetes Self-Management Education and Support (DSMES).
We know managing diabetes can be challenging. Our program provides a supportive space to share experiences, learn valuable tips, and find encouragement — helping you feel less alone.

What to expect
The Diabetes Wellness Program includes 3 group sessions led by a Firefly health guide. Sessions will happen every other week over the course of 6 weeks. Each session will cover different topics related to managing diabetes.
Week 1:
Diabetes management overview
We’ll go over the basics of diabetes, describe the self-care behaviors essential to managing diabetes, and deep dive into two key self-care behaviors: taking medication and monitoring blood sugar levels.

Week 2:
Nutrition and physical activity
We’ll talk about how nutrition and physical activity affect diabetes and strategies for building healthy eating and exercise habits into your life.

Emotional well-being and risk reduction
We’ll talk about healthy ways to cope with the stress of living with diabetes, how to set diabetes management goals (and problem solve when progress toward those goals gets off track), and how to lower your risk of diabetes health problems.
