Welcome to Your Diabetes Wellness Journey
Thanks for your interest in the Diabetes Wellness Program! We’re excited to work with you to manage your diabetes, lower your risk of complications, and support your long-term health.
The goal of this program is to empower you to effectively manage your diabetes and improve your A1c (your average blood glucose level over the past few months), which tells us how well your diabetes is being managed. Our approach complements the care you receive from your doctor or nurse practitioner and is built on the core principles of the American Diabetes Association's Diabetes Self-Management Education and Support (DSMES).
We know managing diabetes can be challenging. That’s why our program provides a supportive space to learn valuable tips and find encouragement — helping you feel less alone.
You’ll get hands-on help to get your diabetes under control and cope with the diabetes management journey.

What to expect
The Diabetes Wellness Program includes two sessions with your Health Guide. Each session will cover different topics related to managing diabetes.
Session 1:
Diabetes essentials
We’ll go over the basics of diabetes, the self-care behaviors essential to managing diabetes, nutrition, medication adherence, and monitoring your blood glucose.

Session 2:
Nutrition, physical activity, stress, and risk reduction
We’ll talk about how nutrition and physical activity affect blood glucose, ways to cope with the stress of living with diabetes, and how to lower your risk of health problems.
